Restrict which payees an agent is allowed to pay at all. Anything outside your approved list is declined, whether the agent got there by error, by a bug, or because it was manipulated into trying.

Choose the amount above which a person has to approve. Routine spending stays automatic; anything material comes to you first.
The rules are recorded with every payment
The policy in force is captured on each transaction as it happens. Change your limits next quarter and last quarter’s payments still show exactly the rules they were approved under. When a payment is questioned months later, the answer is on the record rather than reconstructed.
